Registering for FORMED :

 

  1. Go to formed.org/signup

  2. Select your parish: Enter parish zip code 62454 and select St. Elizabeth and Our Lady of Lourdes from the provided dropdown menu.

  3. Register with your name and email address.

  4. Check your email for a link to start watching FORMED

  5. This is Free for all Parishioners!
